Treatment Materials

• Creating a Daily Report Card for the Home
  Describes in a series of step-by-step worksheets how a parent can establish a program at home to help them better manage their ADHD child's behavior problems and to develop more appropriate behaviors.
• How to Establish a Daily Report Card (School-Home Note)
  Describes how teachers can establish a School-Home Daily Report Card (DRC) for an ADHD child in the classroom to help them manage child behavior and to provide standard communication between teachers and parents.
• Conducting an Outpatient Medication Assessment and Ratings
